Support plan from hospital same as ECHP?

Well, we've seen the multi-agency team at the hospital, a support plan is being drawn up and put in place. I have to agree and sign before it goes anywhere.
SALT, edu psychologist, nursery teacher, SENCO all where there and had input.
I emailed the SENCO about if I can have a copy of DS's IEP for his DLA form evidence (up for renewal soon) and she's referred to the support plan as an ECHP - so which one is it?!
I'm so confused. I thought the ECHP was through the LEA but the SENCO is calling it an ECHP?

ECHP is through the LEA and has to be reviewed annually. You would know if you had one.

No it's not the same but you might want to look into applying for an EHCP though.

I've spoken to hospital & school, they are reviewing the support plan then using it to apply for an ECHP
