I thought I might share something that I found helpful.
A few months ago, I was in the middle of writing my son's Outcomes for Preparation for Adulthood / Independent living in his EHCP and stumbled upon several documents used in the US that are used for their Transition Process. Some of the stuff is very much related to the US, but the rest gave me ideas as a starting point and I adapted them to suit my son's needs and incorporated them into Outcomes and then worked out the Provision needed to achieve them.
