Many years ago I completed my son's first DLA form, which was made much easier by an extra-ordinarily helpful support document that I was linked to by someone here. I need to do it all again, from scratch, and would love to find that document again (ideally, I guess, an up-to-date version of it), but I really can't remember which charity or organisation had it on their website.
Does anyone know what I'm thinking of, and can anyone please direct me to useful help?
