first of all, I have his biological dads permission for DS to use my married name.
I'll be filling out the form for DS' DLA soon. he is 'known as' my married surname at the doctors, nursery etc. so when he was referred for assessment and got his diagnosis all the paperwork says 'DS mumsmarriedname' but his birth certificate and passport still says 'DS biodadsname'.
does anyone know which name will be put in the form? or should I write a covering letter that explains? I will be getting help from a charity to fill out the form so I will ask them but it's maybe not something they've came across before.
has anyone else been in this situation or similar?
