I've found the standard request letter from IPSEA and have used that. School have also given me a Child Information Sheet with various headings and the HT advised me to keep this down to a minimum. However, I'm up to page 9 already and I've not finished. What do you advise please?

I'd advise deleting anything that isn't relevant to the educational need and limiting hugely anything about homelife unless directly relevant to school.

For example, unless there is a direct link to some sort of provision, don't waffle on for pages about your morning sickness in pregnancy, or even preschool. YOu just dilute the important stuff.

Pare it right back and start summarising; nine plus pages is way too long and they will not read it.

You just need important factual information.

You need to get through to them the fact that (presumably) your dc has received lots of help at school and home but still isn't making progress, so concentrate on that issue. If you can quote chapter and verse by reference to P levels or national curriculum levels, so much the better.
