just considering filling it in for dd3 who is covered in eczema, have downloaded the pdf, and its different to the last lot of forms i had to fill in for dd2 (still at appeal stage). they seem a lot more geared up towards behaviour.AS typr disabilities now. if dd2 doesnt get it at appeal then will reapply and thease forms will make it easier.
