Help with conners parent questionaire

Just wondering how I fill out the questions that are school based for ds age 7, eg - has trouble concentrating in class?, fails to complete assignments?, has trouble concentrating in class?, leaves seat in classroom etc I don't know the answers to these do I leave them blank?

also asks if there is sloppy writing or poor spelling, wouldn't his teacher be better to answer this

also have the OT parent questionnaire which asks if there is any difficults in dressing, washing, grooming etc how much details do I give eg ds can dress fine but cant turn clothes the correct way, can brush teeth but dislikes doing so because of feeling and taste

any advice appreciated ive put off doing these forms for a week already

Are you sure you have the correct questionnaire? It sounds like they may have given you the teacher version? If not then just give your best opinion of how you see his academic issues.

no it said parent questionnaire on it and said to answer all questions so just had to guess really, its done now finally
