Guidelines for parents-SA acknowledged letter

Hi, I had the letter confirming they have received my request for SA today. ( I was told by email Tue it had been sent and could I provide any more relevant info so I have already forwarded all relevant reports from professionals that I have)
They have dated the letter 16/09 and said they have 6 weeks from then to decide ( I know they received my letter on 2/9 as it was signed for then and think they only did something this week as I was chasing it Hmm), anyway my question is, they have provided me with a 'guideline for parents form' which is the 'parents questionnaire' I have seen mentioned on here before, it says if they do decide to assess 'if you would prefer us not to ask you for further advice at the next stage and would instead like us to use the advice you are providing here, please sign below'
If a statement is issued your advice will be included in the appendices and may be used in the statement itself.

So I'm thinking I shouldn't sign this bit in case something changes during this long process and I need to amend/add to this information? Is this right? Shall I just make a copy of what I put here and resubmit at next stage with changes if needed?
