Reasonable adjustments duty

Does anyone know much about this duty? I have tried to research on line but am a bit confused.
If a school justify their actions by claiming to have made a reasonable adjustment but also acknowledge that the reasonable adjustment (a strategy) was unsuccessful then does that count as them having made a reasonable adjustment? Confused

Can you give a bit more information? The prospect of an adjustment being successful in removing a disadvantage can be evidence of its reasonableness. If it hasn't succeeded, was it realistic to expect it to? What stopped it succeeding? Can something else be tried?

Its hard to know what you're driving at without more detail.
