Reasonable adjustments duty

Does anyone know much about this duty? I have tried to research on line but am a bit confused.
If a school justify their actions by claiming to have made a reasonable adjustment but also acknowledge that the reasonable adjustment (a strategy) was unsuccessful then does that count as them having made a reasonable adjustment?

Can you give a bit more information? The prospect of an adjustment being successful in removing a disadvantage can be evidence of its reasonableness. If it hasn't succeeded, was it realistic to expect it to? What stopped it succeeding? Can something else be tried?
