ASD - Parent View Form - Statement

I know they send you a form with all the headings but can anyone direct me to either a really good comprehensive skeleton outline filling in all the detail or idiot guide?

I would really appreciate it. My DS is 6, works at a two word understanding and is about to start Y3 next term. I need to write formal Parent View and examples would give me some guidance Thank you x
