They accepted my request for a statutory assessment, yay it has only been 3 weeks since I sent it off too.

Thanks all did anyone else get a little question form with their acceptance letter? Is this standard practice?

You need to be very careful with that form, Crawling. That is your 'Appendix A: Parental Advice' form. Whatever you say in that form, will inform the Statementing Officer of your views about your DD's needs. The Statementing Officer will receive similar forms/reports from Paed/Ed Psych/OT/SALT/Portage/Physio....etc.

You don't have to use the form you've been given. You can do a word document and use the questions they have given as a starting point. It's your document.

Mine ran to something like 40 pages of typed information Blush

Thank you so much Lougle I did try and squeeze short answers on to the form but I will def follow your advice and do a word document as I could write reams and reams more. Thank you.
