Adhd medication help please.

Hi all.

My nephew is 13 years old and he has multiple personality disorder, adhd, bipolar and significant learning difficulties.
He's been on medication for years now And it always seems to work at 1st but wears off after a while. As of 3 weeks ago he has been changed to 35mg of conserta. Since he's been on this his behaviour has deteriorated rapidly.
this is just a short list whats he's been up 2 the past 3 weeks. He climbed on top of a chapel roof to find a bird, he lay down in the middle of a busy road to take a rest , swallowed elastic bands to kill himself .
He is under camhs also.

Has anyone please got any advice, experience or knowledge of what my family can please do next. My sister is at her wits end and we just want to help my nephew feel good again.

I think you need to call CAMHS and ask for an urgent review. What was he on before?

Camhs are aware and say that they are working with other agencies but nothing is being done. The conserta seems to have magnified his issues Nd made them 10 x worse. He's been on ritalin 50mg a day medkinect 40mg rispiradone 1.5 at morning and 1mg afternoon..circadin 10mg.

Go and take him to Camhs with you 2moro and sit there until you get seen ??

Concerta is the same drug as ritalin AND medkinet - methylphenidate, so his dose has been decreased if that's just a daily dose. Is XL in either of the names? Mehtylphenidate comes in instant and extended release forms and the extended release tablets are often labelled XL.

Generally with methylphenidate, there's an initial period of close monitoring, though if he's simply changed dose, they may not be doing that.

If he's putting himself at risk of harm, he does need to be seen and reassessed ASAP, though.

Thank you pipin. It's been an awful month. I didn't know my nephew had all these issues as my sister kept it hidden from us as she was ashamed. She has told me that the police have been called to him 18 times since Christmas and it has no effect on him what so ever. I have never experienced anything like this in my little bubble but now all I want to do is help my family. I have advised my sister to tell his pead that she wants off conserta and i am so sad to say that I too suggested having my 13 year old nephew sectioned under the mental health act so I am so glad you think this could be beneficial pipin. We don't know which diagnosis is the main contributor but at the moment all they are treating him for is his adhd but I feel that it's more than adhd making him this way. Do you have experience In multiple personality or bipolar? Any advice will be a blessing pipin. X

Camhs just keep fobbing us of with the same generic we are working through blah blah blah story. My sister has been there in tears pleading for help but they are not living her nightmare so have not enough time.

Thank you ouryve. There is no Xl in his meds. According to my sister tey have put him on this as my nephew has a fear of needles and won't allow a blood test? I think that this is absurd if true. Conserta lasts for 12 hrs rather than 4-6 so he does not need to take medication in school. I feel that adhd is possibly the least of his worries. Could adhd meds make any symptoms of his bipolar or MPD worse? He is so depressed it makes me feel ill when I see how depressed a 13 year old child is. Earlier he flew in to a rage over something my step daughter had said and it lasted a full 2 hours and 40 mins. He punched doors, walls, himself and screamed every obscenity under the sun but in the next breath he was curled up on the floor sobbing his heart out like a baby. It was so surreal and I couldn't do a thing to help :-(

PipinJo's advice re A&E is sensible, given this extra information and, to be honest, so does your suggestion of having him admitted. Someone has to sit up and take notice, somewhere alon rather than just making tweaks here and there. If he's actively wanting to seriously harm himself, it doesn't need to be glossed over.

Psychosis and suicidal ideation are amongst the side effects of methylphenidate, btw.

And now, i really must get to bed before I can't get off the sofa any more.

When it's been brought up in meeting before they claim he does it because he has no danger awareness and simply cannot see the consequence of hurting himself. They seem to think he's not suicidal at all. My sister said she feels like the majority of the time they are bored of her and they think she makes a lot of it up. It so hard seeing people you love suffering

Yep, if he's admitted overnight for something physical (eg the rubber bands) and they've heard of the self-harm, any children's ward will want a kiddie-psychiatrist to chek him over beige discharge. Which given its a weekend will provably mean Monday.

Thank you all for your help. We spent the whole of yesterday sat in my nephews drs office and we refuse to leave. They are changing his medication from conserta and they are now putting him on an antidepressant and Ritalin so hopefully this will help but the best news is they are now taking my sister seriously and we are having an emergency meeting on Wednesday to see if we can have him put in to a programme to get all the medication out if his system and start from scratch, they can also monitor his behaviour and keep him safe. Thank you pipin for that suggestion. We are so grateful for all mumsnetters help. It was a very emotional weekend and it's the 1st time I've ever seen my baby sister cry. My nephew was very emotional to and begged me to help him alll he kept saying was "auntie please make me feel normal" so it's now my goal to help him. I wouldn't have been able to make the 1st step without all your help though. Thank you so much

Thank you pipin hugs are great fully received at the moment. It with great thanks to the mums (including you) of mn how we managed to move on. I used all advice put it together and they had to listen.

He's under a psychiatrist aswell now so he's being given CBT at a more advanced level? And supposedly they can help his fear of blood tests. He's been put on a low dose of fluoxetine as they were reluctant to give him anything more due to his already fragile mind. He seemed a lot calmer yesterday and he's very tired. He slept for 14 hrs Monday through to Tuesday.
