Writing to the council for statutory assessment

Any advice would be gratefully received.
I am putting together a request for a statutory assessment for my 12 year old son who has been diagnosed with high functioning ASD

Any tips of what I need to put in the letter, I am desperate to get the help he needs put in place

There are model letters on there you can use.

You will need to write to the Chief Education Officer at your LEA (the council office will give you that person's name) and give them six weeks to reply (mark that date on your calendar).
