educational psychologist referral form

My sons pre school are doing one of these forms atm to get help from an educational psychologist a smy son starts school next year. I filled the form in but they need a 332 to send it off. I have no idea what a 332 is does anyone know? Ive rang the paediatrician but shes not there right now so have to wait for her to get back to me.
If anyone can help and tell me thatd be great

A 332 is the form that pediatricians fill in to notify the local authority that a child may have SEN. Some local authorities use them to help prioritise early years children for involvement, sound like you LA is one of them. Its just a fairly short report /letter and hopefully if your child has already seen the pead recently they may be able to write one pretty quickly.
