Statement help please?

So just had my ds's request for statutory assessment of SEN Reply slip Through the letterbox and just wondered if anyone could help with a few q's about it please?
Section 1. Should I add comment? I don't really want to leave it blank tbh but do I just write why I think he should have a statement/ attend a school suitable for his needs?
Or should I choose 'use the evidence submitted with the schools request'?
Section 2. Contacting the following people for a report. Do I add all other professionals involved with DS? Eg Physio, paed, salt etc.

Also, I have read what a individual parental supporter is/does and I don't think I need one tbh, but I don't want to regret not adding one if needed in future (if that makes sense lol). Did any of you use an IPS when doing DC statement?

Hi, I don't think its a standard form across all LAs so tricky to visualise what you are looking at. Are they going to decide whether or not to assess? If so, do you have reports the school don't have that you could refer to and mention in section 1? Def list all contact details for anyone who could provide a helpful report.

Section 2, yes.

Section 1 rather depends what the school have sent and whether you have anything to add. Did you receive a copy of the school's submission?

What stage are you at? Are you just requesting an assessment?

Sorry I didn't make it very clear, very new to this part of it all. yes were just starting assessment for DS's statement he is currently at pre school and has seen ed phsyc and have her report and pre schools report.

Have you spoken to Parent partnership. They were very good at this part in my area.

The things that the LEA are looking at are with the help (if any) currently in place what barriers remain to your child accessing the whole curriculum?

i would say, make sure you send anything you want to be considered, and don't assume someone else will send it/ say it iyswim?
