Just that really. Have got a stiff neck and horrendous headache from ploughing through bloody form, which has COMPLETELY changed since we last filled it in 5 years ago, and now I want to cry because its asking me to fill in a school section. Do I just leave it out and write an addendum? Or fill it in as if I am the school? Which I am.

can't remember even though I renewed at the beginning of the year, will take a look tomorrow at the photocopies but I'm sure I put something in about why school doesn't work due to his needs. HTH for now anyway.

I just sent mine off.
I attached a sheet of paper explaining why we HE and about the problems DD had at school. HTH.

..same as sibling above - sent letter explaining why we HE - with details of how the school ignored the Statement - always cathartic to let people know how we were let down there - and backed it up with all the medical/educational dx's we have.
