I had ours filled in today by an organisation, he filled it in for me, although they may struggle to read his writing!!!

He seemed to use the word inappropriate a lot, I presume this is a word they look out for to get extra marks!!!

He also said that were you struggle to put how many minutes your child does things for etc, he said you can always put 'constantly'

mine for dd2 says 'all day' for a lot of the form.

have to say I had a lot of trouble with the time part of the form so just put 24/7 must have been ok as they extended her
