requesting patient records...who and how to?

Could anyone advise me about how I go about requesting all records for my DS?
I am currently about to be making a placement request to a SS for my DS as MS has completely failed and my DS is in an awful state and we are in limbo and not moving forward.
I do feel I have good support from the school he is at and the Ed Psych is supportive too....but it seems they are more willing (suddenly) to agree ms is inappropriate...whilst of course I have felt this way for a very long time....

What I want to be able to demonstrate is the failures and lack of support my DS has been left floundering by and the passive approach the Psychiatrist has always had and the failure to have picked up or responded to ANY issues, questions I have ever had. That it was only when I took an advocate with me to an appointment, that she finally agreed to refer for asd assessment to be done after 2yrs of asking about this, etc....the failings and issues run deep and there are many...

I am actually wondering if I ought to be taking it all to a lawyer too and seeking to build a case in law v's the education system and medical establishment.....big thing to contemplate but I know I have very good cause to pursue if I decided to.
The main reason at the moment is to compile my case as strongly as possible and have good references to failures and inabilities wioth ms to have ever provided for my DS needs wholistically which has been to his very serious detriment and sufferance...bringing us to where we are now...which is in limbo and seriously floundering, not able to access anything, extreme anxiety and ill health

Do I just phone GP and ask for records, do the same with school, CAHMS...or is there protocol for this and who and where do I start with this?
We are in Scotland if it makes a difference to this too
