CAMHs Referral

Hi, My daughters Paed said they would get school to refer her to CAMHs for review and behaviour help. School phoned them up and said they didn't think she fitted what they would expect for a review but to send in the paperwork and they would do it. School have given me a form to fill in, as I know her best! but its more like an observation form that you'd expect to be filled in by the professional observing her. I'm really fed up with all the forms and don't see how I can fill in something that isn't for a parent - IFSWIM?
Is this a normal form do you know? Thanks

What was the form called? i had to fill in a form at camhs which was aimed at younger children Ds is 7.

Its called 'Every Child Matters Change For Children CAF form - Common Assessment Framework for children and young people (CAF)' and 11 pages long including details about the parent/carer on whether they (we) provide proper living etc...
This is why I find it weird, its for someone to assess what we provide as well as what problems are seen/observed. Surely that isn't for us to complete?

Ive just spoken to CAMHs and they said I shouldn't be filling the form in as its what the school should do. However, as the school do not know the ins and outs of the details, really the Paed should fill it in. They said I can fill in what I know that is relevant, but give the forms I gave to the paed to them along with the Paed letter so that they have all details.

Oh surprise! a school that hasn't got a clue .

No we didn't fill one of those in so i wouldn't of been any help anyway!
