who fills in your DLA forms once discharged from profs

we had a group meeting with ds2's consultant, ot, physio, asd team etc.
Basically the result is that ds2 is who he is, his condition is not going to change (CP, autism & ld's) so other than physio who has ongoing involvement with splints, monitoring w/chair services etc everyone else is going to step back. Great!! After almosy 10yrs of constant appointments we are happy to eb at a stage where we just get on with things.

All fine except when his DLA is up for review (begining of next yr) who do i get to fill in the form.
Usually his wonderful Paed sends them war & piece & i just send them whtever other reports are most recent.

So who else can i use to support his DLA review, not something i even considered at the meeting.
We have had high rate care & mobility since he was about 4yrs old.

Schools are often asked to do the 'professionals' page.

I usually get the school to do ours.

Thanks, the school usually are asked aswell, I guess I'm just going to have to tell my go what he needs to put.
