annual review - do I have to use the school's form?

can I provide the information in a format of my own choice or do I have to use the format which has been provided by the school?

It's up to you.

The LA design their forms for easy processing so if in your own format they might miss something, equally they might pay more attention.

I prefer to do my own because I don't need to waste time on the irrelevant or worse 'leading' questions.

it's not an LA form - the school have provided a format for the child and parent to fill in together.
I don't think that's appropriate for an annual review, although I can see that it would be useful for a progress review or parent's evening.

The ones I have done my own way have had a far better result than the school's form filled in their way. I did keep to the points that they had been asking about and highlighted headings to make if easier for them to flick through and access the points i needed to get across.

I did my own using the headings on the school form. Some areas needed a lot more space than the school form allowed so it made sense. I also prefer to use word that fill in a tiny form by hand!

School send us a form that is produced by Parent Partnership; we do use it as a basis for our report as it makes sure that all the aspects of the statement are covered IYSWIM. We tend to add extra sheets to it as well!
