2 quick Qs about DLA application for DS with CP

DS is 2.5 and has spastic diaplegic CP. I'm filling in the DLA form for the first time and have started it online and the deadline for submitting is tomorrow. I have 2 Qs:

How do I supply additional information when submitting online? I am planning to send in copies of a couple of recent reports and his therapy programme with the application number on it and a short covering letter. Will they consider/match up this extra info with the online application?

I am totally confused by Part 24 - Help the child needs when they go out during the day or in the evening. The Cerebra guide suggests reading this question as where does your child go or where would they go if they had the help they need eg playgroups, swimming, physio etc. It seems a total rehash and repetition of what I have already put for help he needs indoors/outdoors etc. Do I just keep repeating myself?

Please help, getting very stressed about it!

  1. Put on the last notes bit that you will be sending info in by post. Then send it special delivery so you know it has got there.

  2. Part 24. Yes you put all of the activities that you do and what help they need. Also what activities you would like to be able to do and what help they need. You repeat yourself all over the form.

I suggest putting in a diary of your day so you can show how much extra you do for him. I did this in the extra information section.

Yes it is very repetitive! On mine I out help with softplay as she won't go in if other kids there, help with swimming as she dislikes the echo, at home help with reading and art blah blah. You need to point out the extra help he needs compared to his peers. Yes post in extra info with your app no on it

I also read part 24 like ben: I put down the extra help DD needs for all kind of activities and also what kind of help she would need for other activities but which we cannot do as we cannot facilitate the extra help needed iyswim .

if you have not looked at the cerebra guide, maybe worth having a look (even though deadline is looming). cerebra guide

DS is 5 and has autism.am trying to fill in the dla form but its avery difficult form.is there some one who can help me fill it in. i have so much information but i just dont know what to write.

Sushinetoday - you may have a local welfare rights or autism charity which can help you.

However I used the Cerebra guide that chocjunkie linked to and was successful first time using their advice. It takes you through each question step by step. If you have any queries we are always happy to help on the board,
