Ed Psych Agreement Form - Any advice gratefully received!

DD's Nursery are in the process of applying for a Statement for my DD who is partially sighted. I've been given an 'Agreement for EP Involvement Form' and I need to fill in the following sections:

Please indicate your main area(s) of concern about your child

and

What outcomes would you like to see? Do you have any ideas about how we can help achieve these?

I'm new to all of this and I don't even know what the Ed Psych service does to be honest. I've already make a huge mistake with school placements so don't want to muck up the Statement request too. Advice is sparse and after the school placement fiasco I wouldn't trust anything the LEA said anyway!

So, if any of you lovely people have any advice on filling in the form, what to make sure I say and what not to say I would be very grateful.

I think you just need to answer the questions honestly - its just your opinion, but give as much info as possible.

We filled out a similar form. I think I put my main areas of concern as DS's sensory issues, DS's ability to be involved in lessons and his ability to understand the work given with his fine motor and cognitive problems.
