Anyone?
Just got the form in the post and a bit overwhelmed by it!
If you have done this - how did you get quotes?, a medical reference?, figure out mandatory costs? and additional costs?, did you get LA to write stating they refuse to provide these items like it states on the form? We have never been seen by SS, are not under any paeds or clinics or OTs, etc. Have emailed GP's surgery to ask if they would write reference.
