HV cant sign dd2 dla forms.........

as he is leaving today, and he doesnt see dd2 enough either. BUT he is referring her to CDC, we should have an appointment by the end of the month. we are offically in the system now. phew.

sparkly is going to sign the bit that about knowing the child well, she sees her a couple of times a week.

will put the HV down to tell them about the differculties if SENCO at pre-school cant do it.

Have you asked the SENCO? I'm pretty sure that the report ds SENCo wrote made a big difference with ds getting DLA.

SENCo wont sign as she is actually still a trainee, tho i think she will be ok with being contacted.

senco wont agree to being contacted eihter. ffs, the preschool are the main team of 'professionals' who see her more than anyone else. what am i supposed to do?

surely in her role as a professional at the pre school she would have to agree to be contacted?
it's in the best interests of the child.....

she wont do it, as she is in training and her bosssays no. they gave mieow the same line when her dd1 went there and she needed someone to sign. HV (new one) should be round in the week at somepoint, and i will makesure dd2 is there, t least then they can say they have seen her on xx date. i am making it my mission to get dd2 as in your face as can be.

if i can get an appointment i will, if new HV cant do it. again the GP barely sees her.

why does her boss say no?
is the boss the leader of the pre school?
surely it is in your dd's interests and theirs for them to be as helpful and cooperate as possible...
i would have been angry if pre school had refused to do anything for me when my dd was attending(in regards to forms/liasing with other professionals)
that imho is part of their job

they say they will liase with Full SENCo assessment as they want all the help in palce ASAP but wont do the disability forms as she doesnt have a disgnosis yet.

ASk your gp misdee, It looks much better coming from a professional. My ds's gp was happy to sign even though she rarely saw my ds, I gave her a report which she used to gain info on ds's difficulties. Would you be able to do that?

but she doesn't need to have a diagnosis(that so infuriates me misdee, when dd wasn't diagnosed no one wanted to "know her" and i fought tooth and nail but was still encountering a brick wall, just 2 days after a major row with local lea a cancellation appointment for consultant neuro pead was given to us and then on day 3 when i phoned them back after the appointment and said right we have a diagnosis---guess what amazing they wanted to "know her" and start putting things in place)
on the form you can put
?(suspected problem)...
it sounds like they want the help for themselves so why would they not be prepared to sign a form that will allow you to get help?

consultant did ds2 forms for me at 7 months old
he has still no diagnosis and he is nearly 9

waiting for aped appointment as well. but that culd upto 6months before she gets seen. hopefully the new HV will sign when she brings the forms round about CDC for me to sign.

I put our GP down as someone to contact. I have never been to see him about ds (except when poorly) I saw one of his colleagues. I went to see him before i sent the form and talked to him about ds and applying for DLA etc and gave him a summery of ds problems as I saw them. He was happy with this arrangement and said he would fill out any forms that the DLA sent him.

Ds2 has no official dx yet as the Paed still won't put it into writing. No-one refused to sign the DLA forms though and ds2 was awarded higher rate. IMHO it's madness to say that they won't sign because there's no dx.

Right then, If new HV wont sign them as i only want thm as a contact person IF DLA wantto contact them, tho the forms are chock a block with info, theni ill go back to the GP again to see if she will consent to be contacted.

well i have just sent off DD1's DLA stuff, i got a good friend to do the person who knows the child well bit and i put her TA down as contact from school do you think that will be o.k, i could get the headmaster to do it only he doesn't know DD as well.
