Applying for a statement - what do I do??

I was originally going to let the nursery, paed etc etc build a case up of ds and then apply for a statement with their support but then realised I could be waiting forever so just going to apply independently now. What is the most straightforward way of me doing this? Do I just write a letter or is there a form online that I can download to fill in? How does it work? Where do I send it?
