DLA form section part 36

I have to renew DLA and am struggling to think who to put in at section 36 (statement from someone who knows the child). My DD is 13 and has ASD.

All the guides say to ask a health professional but DD hasn't seen anyone for ages. Was discharged from SALT and CAMHS years ago, and only contact with the GP surgery is the annual check for asthma and a flu jab.

The only other people to ask would be SENCO or form teacher. Do you think this would be ok?

I got DD's nursery teacher to fill hers out.

We just usually ask a relative, which I think is accepatable iirc - the advantage of having a professional is that it is one less person for them to contact.
Make sure it is someone supportive.

I got the SENCO of DD's nursery to fill it in.

I filled in that section for the child I support, I'm his statemented TA. I got the advisory teacher to fill in my DS's.

Headteacher filled in dd's. He knew exactly what to put too without any prompts which I thought was impressive.
