Just a quicky: Do you send any supporting info with the form as there is very little room to explain how your child's behaviour is affected at home, in school and out and about? I know that they normally do a home visit anyway so do you just give them the extra info then?

No dont send, I just did our first application and only sent the form itself. They had a quick look at papers on the visit but it was mainly finding out about our day to day tbh. Lovely staff. We got hol vouchers a week or so after.hth

hi. I didnt send it but showed it at home visit. A week or so later recieveed holiday vouchers and funding for a carpet for ds. hth.

Thank you ladies. One FF form in the envelope then.... Fingers crossed as we probably won't qualify for tax credits next year when they change the limits again so hopefully will get an award this year :)
