I have to fill this in for next week. LA have asked their EP and S&LT and our Head.

I'm going to put our EP and our S&LT. Should I add anyone else? For example, a teacher or TA from school or the NHS OT who is treating DS. OT evidence is agreed but she knows his difficulties better than most.

Also, if it proceeds to Tribunal I may instruct lawyers - should I indicate the name of the firm on the form even if they are not as yet instructed?

No, don't worry about the lawyer thing yet.

You don't need anyone else to attend, but you might like to consider whether the class teacher, or TA or anyone like that could be useful to you.

Generally they are a bad choice if likely to come out on the side of the LA but sometimes they can surprise you.

It kind of depends on what it is you are asking for too.
