Posted the claim for DLA today!

Have finally after 3 months of filling in forms and gathering appropriate evidence completed the DLA form and posted it :)

DS has GDD, ADHD and Autism.

I included reports from:

Class teacher
CAHMS worker
Paedatrition
Speech and Language Therapist
Continence Nurse
Family Support Worker
Social Services

All of which say more or less the same thing so hoping it is enough evidence to be awarded without numerous appeals.

i didnt send any reports as they contact them anyway but was awarded mine without having to appeal so if youve sent all that already im sure you will get it first time so good luck

I am about to post off Ds2's application after dithering and putting it off for nearly 18 months 8). However, I got the form from the JobCentre and have no idea what address to send it to. Please could someone tell me where I need to look for the address? I know it's part of the DWP. but that's about it.

Go online and find the dla support number on there are ask them.....? Mine has to be posted to DWP in birmingham, but that could be becuase we are in the west midlands.

We got ours turned in at the end of January. Called them today for a (silly) hopeful update, and was told the blanket answer of "takes 11 weeks to process claims, we'll let you know if we need anything else.." yep. not horribly helpful or positive... Hmm

tHANK YOU SparkleRainbow. It's occurred to me it might be the same as the address from the DLA letters I get for Ds1.

once youve done it once (and i have a disabled DS and DH so i have two to do Confused) its not too bad you get used to it, one thing i will say is make sure you copy each page so as next time its so much simpler to fill in esp if nothing much has changed and you remember HOW you discribed things an its not so much like your starting afresh you then when you do the next one can do it quite fast and just add any new additional information and dont put it off your losing money i know its horrible but so worth it put the kids to bed have a glass of wine relax and take your time gather everything up you need to send and take a deep breath

Heard back today ..... it was awarded !!!

We have higher rate care and low rate mobility, more than we thought we would get and no messing about with medicals etc.

I'm delighted!

Wish i'd read betty's post earlier as didn't copy the forms and would have been handy, how often do you have to re-apply? The letter dosn't state how long it's been awarded for.

Fantastic, rockmaiden! From what I remember you need to apply again at 5, 10 and 16 years old? I may be wrong, someone will know.

I photocopied mine-but we get sent so much paperwork have no idea where

it is anymore-maybe in one of my carefully filed boxes....in the attic?

My son's paperwork is the only thing organised in this house, it has it's own filing drawer.

We get sent a lot and it seems that ever therapist we see has lost / not recieved some piece of information or another so forever copying and giving people various bits from it, just never occured to me to copy the DLA form.

Son is nearly 7 so does maybe will have to re-apply at 10 then, that's not too bad. Was thinking it might be yearly which would drive me mad.
