DLA help again

I ahve just spoken to dd's pead. and she reckons I should send in the whole of her annual reveiw thing as it covers medical/therapy and education
has anyone else done this for supporting information?

I sent all of DD1's statementing stuff.

dd3 wasn't statemented last time we did the forms but as I now have a shiny new set waiting to be filled in I will be sending statement stuff in. Postman might want to get a reinforced van!

I sent in anything which I thought my help with my claim. This included diagnosis report, letters from SALT, letter from school, IEP etc!

the form shouldn't be too hard and tbh it is all 24.7 stuff.
ok I am dreading filling it in.....I will do it tomorrow
