That bit when you have to say how many times in the day a child needs more help than NT child of same age and then say how many minutes ?

What is the required form of words ? I did photocopy the last form but now cannot find it ! But I know there is a formula to follow. I am describing day time assistance not night time so all help is for 12 hours ???

I put 12 hours and just wrote frequently throughout the day. HTH

I also put frequently throughout the day. I think you can ring the DLA and ask for a copy of your last form which was successful.

Mine gets peppered with phrases like "constantly " or "all waking hours".
