Letter from DWP - request for additional information

Have received a letter this morning from the DWP saying they have written to my daughter's school for further information. She's been in receipt of HR DLA for 10yrs and has recently transferred to a SLD school, so can only assume that this is the reason for the request. Does anyone know what kind of information they request (ie a standard proforma or something more personal)? Thanks for your help.

From memory there is a standard form asking the school what type and level of support they think the child neeeds in different areas eg self-care: changing for pe, eating, social interraction, medical needs, support in class and in unstructured times also any health and safety issues. Hope this helps.

I'm Pretty sure it is a standard proforma - there is a question on the main DLA form asking yout to give a school contact; so they will be sending the form to that person.
DMight be worth you contacting the school and having a chat with them if you are on good terms so that you can make sure they are on the same wavelength as you.
