Ehcp case review form query

In this it has 4 boxes to list outstanding issues, the reason for the appeal is a refusal to issue from the LA. Should I just put this in one box and leave rest blank or add placement needed etc?

A refusal to issue appeal will not consider placement. That is outside the scope of the appeal. Yes, just use one box to state the outstanding issue is whether an EHCP should be issued.
