Dla question 24

Hi
First time filling out dla doctor told me to do it as child is on waiting list to be seen by paeds and to be assessed for asd and adhd they are currently in nursery and under speech and language. I’m stuck on this question as he hasn’t got any diagnosis or reports.. would it be better to wait and not do the dla form until I have these?
im currently on question 24

Hi, I haven’t long done my daughters Dla form, she’s nearly 4 and was awarded middle rate care, she is waiting to be seen by paediatricians for asd / adhd signs too, on that question I put no and sent it in then later if you do get anything while your waiting send that in as evidence, with hers I sent in a letter from preschool as there person who knows her well, a screen shot of repeat prescriptions ( she has problems releasing her bowel ) and a letter from her bowed leg appointment. It takes around 17 weeks before you hear a decision, so you will have plenty of time to send it extra evidence if you receive any but don’t worry if you don’t have any just fill out the form stating your child’s needs in detail

Don’t delay sending the form, but make SARs to get copies of the paeds referral letter (and the referral letters for ASD and ADHD if they are separate referral forms/letters), information from SALT and information from any other professionals involved. Do you have any information from nursery?

@Hayleyb5425 it depends on the individual child rather than the diagnosis.
