Is an assess plan do document the same as an IEP?

I am drowning in trying to complete an DLA form for my DD. It asks if my daughter has an IEP. She has an assess plan do review plan with the school - is that the same thing? Not sure what to tick/include.

Please help! My goodness - it is truely awful completing this form. Both the amount/detail they want but also listing everything out every single thing they struggle with 😒
