Incident reports

Does anyone know if all behaviour incidents should be recorded by school, for example a huge meltdown by a child.
My sons school do not seem to be logging such behaviour incidents and with one of these his safety was put at risk

I don't know if they legally have to but my ds school has a recording system called cpoms which they add any incidents/concerns to. I also worked in a school and would recommend any significant incident so this would include injury, bullying, anything that required a time out or one to one supervision. Is your son being properly supported?
