Hi all - new on this board and wondering if you can help? I'd like to make a Subject Access Request to my son's academy as I'm finding it very difficult to get information about what provision my ds is actually receiving. Not sure how to go about it. I can't find any information on the school's website...

Hello, just read your thread. Could you perhaps ask the SENCO, they should be able to get this information for you

Just write to the Data Protection Officer, c/o the Head Teacher (email is fine) and ask for all your son's information, including reports, emails, anything held on a computer, any paper files etc.
If your son has input from an NHS SALT, you will have to submit a separate SAR to the relevant NHS department - might be part of a local NHS trust.

I'd also do a SAR to your LA too, asking for all info - if you google your LA's name and Data Protection, that should give you the details for that. Otherwise, again write to the Data Protection Officer, C/O the Councils' Central office.

If your son is 12 or over, it might be an idea to tell him what you are doing and why and ask him to sign to give consent to submit a request on his behalf - and send that in too - just a simple note saying he agrees for you to submit it.
