Dyslexic and ADHD assessment

I am looking to getting a dyslexic and ADHD assessment for my 11 year old who struggles with maintaining attention and retaining information.
How do I start going about getting an assessment for him?

If you haven't already set up a meeting with the school SENCO, they can put a referral through as can your GP. We chose to go down the private route but still required a referral letter from the GP. School were fully on board and supported us throughout. Hope all goes well.
