Anyone with EHCP request form experience?

I am in the process of filling out a form to request an EHCP Assessment for my Dyslexic DS. However, I am so worried I will not word it correctly or miss out vital information. I have spoken to IPSEA who gave me some pointers but of course unless anyone has all the paperwork, assessments etc in front of them its very hard to give information over the phone. I feel quite disappointed that we are left alone to do this, however I cannot risk letting school do it as they will not get round to it for weeks and I am then dependent on them putting everything in. Has anyone ever written one? Can anyone give me any guidance?

I haven't but will probably doing similar later this week so I will also be watching for advice.

I have drafted mine it has taken me over a week. I have read and re-read the SEN Code of Practice and referred to it and also the own Councils Dyslexia Policy. I am waiting on information from of course, school, and then I think I will just have to send it off, with all the assessments and reports.
