2 year old sleep problems

I'm struggling with my 2 year old waking at 1am most night and will not go back to sleep unless I take him in the spare bed with me. He has always been a bad sleeper and had reflux since born and had cows milk allergy so I've had to rock him to sleep since born or sleep with him on and off. Before Christmas he was waking through the night on and off but would settle after rubbing his back etc now he just won't settle and cries until I pick him up and take him to the spare room, sometimes that doesn't even help and he will cry in the spare bed to. He will always have random days where he will sleep through the night without a problem but I'd say once a fortnight I get lucky with that happening ha! He still naps in the day and I have tried reducing and extending but it doesn't help, I stopped his nap once and he was a nightmare in the evening crying constantly because of tiredness. It used to be that I would only sleep with him in the spare bed when he was poorly but now it's every night, I wanted another child so he can have a sibling but I don't know how I would cope with two through the night and working in the day!

From my experiences I wonder if you're inadvertently fueling the wake up. What happens if you just ignore him? Are you keeping things boring enough?

He cries till he is sick if I ignore him, I have guilt so bad for making him this way but I can't think of anything else to do because he cries so much he starts coughing then sick and also if he isn't sick he becomes so awake he won't sleep no matter what and just cries constantly which doesn't stop until about 5am then I cant wake him at 7am. I have tried giving him a drink of milk when he wakes also to try and make him sleep again but doesn't work.
