PLEASE HELP! Early morning / night time wake ups

PLEASE CAN I HAVE SOME ADVICE? GETTING DESPERATE
Any tips to help combat night waking / early mornings? DD is 5 months and any semblance of a routine is out of the window as she’s begun waking for the day at 5-5:30 (I do try to settle her but she’s wide awake).
She also often wakes at least hourly from around 2/3am onwards. This has been going on for ages so isn’t a leap.
I’m nearing the end of my tether with sleep, she’s worse now than when she was a newborn.
Although daytime routine is non existent, I tend to follow wake windows, so she will have a nap 2-3 hours after she has last woken up, and generally doesn’t sleep more than 2.5-3 hours max in the day.
Bedtime routine is good and she can self settle (although not always!). She is usually down and asleep by 6:30-7pm and dream feed is at 10pm.
She is EBF but has - large bottle of expressed milk after her bath and again at the dream feed.
How do I stop the middle of the night wake ups? (I of course feed her if she's hungry but I don't think she is hungry every time she wakes! I feed her the first time but when she wakes 45 mins later I don't think hunger is the issue).
And how do I stop the early morning wake ups?
Grateful for any advice....

Following with interest as my 6mo baby is exactly the same, except the hourly wake ups have been from 10pm this week. Sending sympathies, it’s shit and exhausting

My 11 month old has been like this for months. Sometimes she wakes as early as 11pm, or 1am but usually from 3 onwards. I co sleep with her from the night wake and then she sleeps til she gets up. She has started to sleep til 6/6.30 but the night waking has stayed the same. I've never been able to fix it, I think its normal. Apparently the latter part of the night is light sleep for them so the only suggestion I could make is co sleep and go to bed early. But it is a shit (hopefully) phase.

Following in case anyone has cracked this. My 8mo wakes at 4am every day and nothing I’ve tried has made the smallest difference.
