Tips for transitioning from co-sleeping

I'm trying to transition my 6mo to sleeping on her own. At night she wakes after 5 minutes if she can't feel me by her. That's if she's left on the mattress or if I transfer her to the next2me.
She'll sleep for 30 minutes for a nap if I pop her down but won't do it at night at all.
Has anyone had experience with a similar baby? Is there anyway for me to gently teach her to sleep on her own or am I fighting a loosing battle?

No help but my 15 month old is the same x

To gentle teach her you need to accept that it's a long, slow journey to get there.

For the first step, start to slightly roll away from her after feeding lying down, but keep your hand on her chest. The idea is to get her used to the closeness and knowing you are there by your hand being on her chest, rather than your torso at her side.

A dummy may well help too.
