3.5 year old wetting themselves!

My 3.5 year old nephew has been dry day and night for about a year now, but recently he keeps wetting himself. Sometimes it's a whole wee and sometimes it's just dribble then he says he needs to go. He says he doesn't know he's doing it. I'm wondering if this is a phase or could be a UTI or something else?
My problem also, is that although my sister isn't abusive or anything like that, she isn't exactly attentive and basically can't be bothered unless her kids are really ill. They practically live at my mums! She's been told by his nursery (about 6 months ago) he also has a problem with his ears that needs checking and she still hasn't taken him to the doctors!!
Is there any way I can take him to the doctors or do you have to be a parent or guardian??

Your mum might be better taking him to the doctors. If he is left in her care, then she could reasonably say she's caring for him while his Mum is busy so bought him to the doctors. Best not done behind the parents back tho, tell her you have arranged it.

It's unlikely you'll get anywhere with docs at that age I'm afraid it's still very young.
