14 wk old lost ability to sleep.... HELP!

Help! For the past week DD's night sleep has been getting steadily worse (naps have always been bad). The main problem is that she can't seem to go back to sleep. Last night she was down at 7:30 then awake 11:30-12:30, 2-3:30, 4:15-6, 7:15-8:15... Got her back to sleep by combination of bf, dummy, holding her hand (she's in crib next to our bed), in arms and bringing her into our bed. Until a week ago she was great - generally sleeping 11;30-6/7 and if she woke in the night I'd pop a dummy in and she'd be straight back down. She has always needed bf or dummy to get to sleep though....

I'm only managing about 3 hrs of broken sleep and I'm desperate to do something about it. I'm hoping it's teething, injections, growth spurt etc and temporay but am really worried that she's just got the wrong sleep associations and they won't work any more. How cn I work out what's wrong and what shall I do?!?!?
