9 week old fighting sleep... any advice?!

Hi guys,
I'm wondering if anyone can offer advice/reassurance to a frantic first time mum!

My baby is 9 weeks old, he sleeps really well at night (10-7:30ish just waking up for feeds) but we're having serious issues getting him off to sleep for daytime naps and night time sleep.

I can tell he's tired after about 45 mins - hour of wake time, he will rub his eyes, yawn and then get grouchy (very quickly!), I try getting him to sleep before he is grouchy/crying otherwise it's a complete mess but even when he's only just started doing tired cues he will really fight sleep. I usually hold him and give him a dummy and give him a gentle rock/sway his eyes will start to flutter shut and then he will jolt awake and scream... calm him down, eyes will flutter shut and then again he will jolt awake screaming, this can sometimes go on for hours until he passes out from exhaustion! I don't know what to do... is this just a developmental phase I need to ride out or are there strategies I can try?

I use a love to dream swaddle and it seems to help my little boy (also 9 weeks).
