Cat naps and early wakings

I’m having some troubles with my LOs sleep and not sure if we would benefit from one of the programs.
She’ll be 5 months in 2 days and is exclusively breastfed. A poor day time napper. Day Naps aren’t non existent but they’re quick, 20-40 mins 3 times a day, can sleep for 2-3 hours if out in pushchair.
Bedtime is between 7&8. She can fall asleep on her own but will wake 40/60 mins after being put down. The only way to settle her and get back to sleep is to put her on the boob. She usually falls asleep within 5 mins. This process repeats hourly until about 11.30 where she is capable of sleeping until 6.30am.

I’m at the end of my tether as I get no ME time at all :(

Has anyone experienced the same?
How can I get her to nap for longer in the day?
How can I stop her being dependent on the boob?

I've considered weaning early in case she's too hungry but she can't even sit up yet
