1 year old cannot get back to sleep - schedule? Training?

My 1 year old has never been a good sleeper, we've always struggled with early evening wake ups no matter what her age/what we've done.

We have just followed Jay Gordon's weaning approach which was great for a couple of days with tiny wake ups and no feeding. But now we're back again with the usual - waking around 9.30pm and despite her best efforts, she cannot get back to sleep. She'll try for 30-60 mins with us rubbing her back etc, then she's overtired and moves into heavy crying for further hours.

This is our schedule - am i getting something really wrong? HELP, it's been a year of no evenings :-(

6/6.45am - Wake up
7am - Feed
8am - Breakfast
9.20 - 9.50/10 - Nap 1
11.45 - Lunch
1/1.30 - 3/3.30 - Lunch nap
3.30pm - Milk
5pm - Dinner
6.30pm - Milk
7/7.15pm - Bed

9/10pm - Wake up every night. For 30mins - 3 hours, depending. Used to feed but useless, so stopped.
2/3am - Wake + feed

She goes into her cot awake, no feeding to sleep, and she self settles well whilst holding our hand. She goes to sleep well at the beginning of naps and bed, but once she wakes up, thats when we struggle.

Any help would be appreciated. Thank you
