Umm, I had one of these and she didn't sleep for more than 30 mins at a time until she was about 8 months old. Don't know what triggered it, unfortunately. She also used to stir a lot in the night and I've come to the conclusion that she's just one of life's light sleepers. This is no help whatsoever, but just wanted you to know you have my sympathies.

Twiglett, dd went through a phase of only doing short naps in the day at that age. But she did sleep very well at night, it was bliss, she did 12 hours in total at night and would do maybe 3 45 minute naps in the day, usually in the pram. At about 8 months old we started putting her in the cot in the day and she'd do 2 1.5 hour naps but then slept a bit less at night. I don't know why it changed at that point but hang on in there and if she seems to not be tired or grumpy she's probably getting what she needs for now.

Twiglett,
Were the same. Each day goes something like this.

Wake 7AM, Back to bed until 9.30, Nap at 11am for 30 mins.
The rest of the day she naps in the car seat or on my lap for about 10-20 minutes.(its what I call power napping)
In the morning when shes in her cot she wakes after 30 mins and if I leave her she doesnt even cry she just plays. Ive also tried putting her down drowsy but she still wont sleep, again she will play for 30 mins before getting fed up and crying.
I received a pampers sleep leaflet yesterday which says that babies at 16 weeks should be sleeping 12 hrs at night and 4hrs during the day, they certaintly didnt take my dd into account when they researched that
Ive tried everything to try and get her to sleep more but to be honest most days she seems perfectly happy just to have the od 10 mins here and there. Ive come to the conclusion shes a light sleeper who doesnt need much sleep. Shes also started waking at 4am to play in her cot for 1-2hrs.
Sorry I dont have any 'how to fix it solutions'
