5 year old talking/shouting/crying in his sleep

I'm just wondering if anyone else has any experience of this or can offer any advice.

He has never been a great sleeper, though partly because he had to wear a surgical appliance at night until he was 4.

He often talks in his sleep, but on nights where he has gone to bed late and has often had a day of excitement the sleep disturbances are awful.
He will really scream and shout and thrash about. He is partially responsive but not often making sense. We can't rouse him at all.
On the nights he does this he will settle for 20 mins or so and then ten mins of noise and this will often go on for hours.

I always thought he would just grow out of it, but we are at the point where he is wanting to sleep over with friends and I can't really expect other parents to deal with this. He also does it when he stays with my parents and although my mum is fabulous with him, it is incredibly tiring for her and it means that we're having to turn down stuff we want to do as we can't be away for more than one night.

So does anyone have any advice, or do we really have to wait this out?
